Who does nipple inversion affect?
Nipple inversion may affect women in the following cases:
• Congenital: Due to developmental abnormalities or short central lactiferous ducts.
• Acquired: Due to breastfeeding/multiple pregnancies, infections (e.g. mastitis, duct ectasia), trauma/scarring, breast ptosis, significant weight loss, or more rarely malignancy (such as breast cancer). Important: Sudden and unilateral nipple inversion requires immediate medical evaluation, as it may indicate an underlying condition.
What are the grades of nipple inversion?
Nipple inversion is classified into three grades depending on severity and duct involvement:
• Grade 1 (Mild): The nipple can be easily pulled out with simple pressure around the areola and often remains protruded. Breastfeeding is usually possible.
• Grade 2 (Moderate): The nipple can be pulled out with more difficulty but retracts once pressure is released. Breastfeeding is often difficult or impossible.
• Grade 3 (Severe): The nipple is permanently inverted and cannot be everted even with manipulation. Breastfeeding is impossible. This grade is associated with significant fibrosis and may cause hygiene-related skin problems.
How is nipple inversion treated with plastic surgery?
Correction of nipple inversion is performed through a simple and safe surgical procedure aimed at permanent projection of the nipple. It is usually done under local anesthesia and does not require hospitalization. Dr. Floros applies individualized techniques, which may include:
• Release of fibrous bands/lactiferous ducts: Through a small incision at the base of the nipple or areolar border, the fibrous bands and shortened ducts pulling the nipple inward are released. This is the most common technique, but it results in loss of breastfeeding ability.
• Preservation of lactiferous ducts: Special techniques are used to stretch and reshape tissues while preserving the ducts. This approach aims to maintain breastfeeding function but is mainly indicated in mild cases.
• Use of local flaps (with minimal visible scarring) or combined techniques.
The nipple is then secured in its new position with specialized sutures.
